02 - FILL OUT THE FORM
Fill out the form completely and correctly. After you have finished filling it out, please check it again to avoid any errors in filling it out.
03 - TTD PA
After filling out the form, then ask for your Academic Supervisor's signature.
04 - SEND EMAIL
After being signed by the PA, the files are then sent via email to: aktuaria@sci.ui.ac.id
05 - PROCESS
After the files are sent via email, the files will then be processed by the Study Program.
06 - THE RESULT ARE SENT
Once processed, the files will be sent to the student.
08 - DONE
Results are provided within a maximum of 5 working days.
